Insurance, Fees, and Payments
Kwan Acupuncture & Herbs is not an in-network provider with any health insurance plan. We provide out-of-network services only, but it is the responsibility of the patient to determine if they have out-of-network coverage. If you have out-of-network coverage for acupuncture then we will provide you with a detailed, paid bill to submit to your insurance company for payment directly to you (this is called a Superbill).
All patients are treated as self-pay patients and billed in accordance with our self-pay fee schedule. Please call for information on that fee schedule. Fees are collected at the time of your appointment.
New Patients
Our practice management software, UnifiedPractice, uses a Patient Portal system for new patient on-boarding forms. You will receive an email asking you to set up your Patient Portal account. Please click on the box that says “Create Account”. To create an account, all you have to do is enter your email address and create a password. Any questions or concerns please call our office and we will provide assistance.
Office Visits
An office visit can be for an evaluation, consultation, and/or treatment. An office visit fee applies to:
(i) new patients;
(ii) existing patients that have not been seen in the past 6 months or more; and
(iii) existing patients presenting with a new complaint.
Office visit fees represent the time and effort required to ensure we understand your chief complaint, medical history, and that we provide the best possible service.
Appointment | Cancellation Policy
Every available appointment time is a valuable opportunity for a patient to relieve pain, alleviate suffering, and commence or continue healing. Please be considerate of this and cancel appointments that you will not be able to make at least one business day in advance. Cancellation of appointments less than one business day in advance are subject to a $75 cancellation fee for exisiting patients and $150 cancellation fee for new patients.
